We’re now in-network with several major insurance providers, including:

Ameritas, GEHA, Principal, United Concordia, Careington, Sun Life, Blue Cross Blue Shield, Anthem, Manhattan Life, CareFirst BlueChoice, and many others.

We strive to make it easier for our patients to maximize their insurance benefits across a wide range of specialized periodontal services — including gum grafting, periodontal surgery, dental implants, and more — with a focus on preserving natural teeth and improving long-term oral and systemic health.In order to best assist you, it is important that you submit your dental and/or medical insurance information to us with your New Patient Paperwork.

Should you be diagnosed with treatment services, we will be able to provide you with a detailed plan which will give you an estimate of how much your insurance company may pay for each procedure. Our business team will be happy to help you understand your policy.

Professional care is provided to you, our patient family, and not to an insurance company. The insurance company is responsible only to the patient as a benefit from the individual or employer plan purchase, and the patient is responsible to the doctor. We will assist you every way we can in filing your dental or medical claim and handling insurance questions from our office on your behalf.

For our patients without insurance, we also offer the ROOT™ Loyalty Program, a cost-effective savings plan that starts at just $49/month and includes (but not limited to):

Patient Appointments

Our practice operates by appointment. We strive to maintain punctuality and kindly request the same consideration from you. If you need to reschedule, please notify us promptly, preferably with at least 48 hours' notice to avoid a $50 cancellation fee. We understand that unforeseen circumstances happen and appreciate your cooperation in this matter.

Our office strives to schedule appointments at your convenience and when time is available.

Financial Policy

In an effort to reduce the cost to you, the patient, while maintaining the highest level of professional care, we have established the financial policy as follows:

Payment is due at the time of service.

We gladly accept cash, check, Visa, Discover, American Express or MasterCard payments

Payment plans are available through CareCredit, iCreditWorks, ProceedFinance, and Cherry

Appointments & Consultations

We are happy to schedule a second-opinion consultation with our doctors to discuss treatment diagnosed for you. It is important to us that our patients are committed fully to a recommended course of treatment, and we make every effort to provide all available information on the types of treatment completed at ROOT™ Periodontal and Implant Center, as well as options to make appointments comfortable.

All patients that are referred to our office from another practice, or who have self-referred themselves for a second opinion are required to undergo a complete examination by our doctor before they provide their diagnosistic opinion. Please ensure that all available x-rays and chart notes from your prior office have been transferred to us prior to the scheduled appointment for review. Please note that many offices require 7–14 business days to transfer records.

It is imperative that we have all available dental (and medical if related to trauma or medically diagnosed disorder) insurance information in a timely manner, as we will be providing you with a comprehensive treatment estimate after your consultation with the doctor.
